Your first visit begins with a comprehensive evaluation regarding the specific services you are considering. If you are moving into the area please ask your former doctor to send a copy of your records. Dr. Cohen will develop a treatment plan to address your specific needs. Please feel free to ask any questions you may have about your treatment options when you come into the practice. When you come in for your initial visit please be sure to bring your insurance cards, referral card and any important information from your doctor.  Please arrive early to fill out your initial paperwork if you have not already done so.  If you are on any medications, please bring a list of them with you to the office.

Please be aware that our practice performs medical and surgical procedures. The forms that you are asked to complete include your medical history which is of the utmost importance to provide you with safest, best possible care that our staff may provide.
